If you need an address in Holmes County

You must submit an address request form to the Holmes County 911 Coordinator for approval.

After downloading the form:

1. Print a copy of the form so you can fill it out
2. Answer all the questions, don't leave anything blank
3. Fax your completed form to

If you do not have access to a fax, you can bring the completed form to the 911 Office at:

809 South Waukesha Street.

(Located at the Police & Fire Station next to Memorial Field)

There is a processing fee for all applications for addresses

The Current fee is $20.00 payable to "Holmes County 911"
